Lesley Campbell, BA, Targeted Case Manager (TCM), plays a key role in coordinating care and services for patients, particularly for individuals with complex needs, including those with mental health, substance use, or chronic medical conditions. She is committed to supporting individuals and their families through every step of their healthcare journey.

Lesley works closely with individuals to assess needs, develop individualized service plans, and coordinate access to medical, educational, social, and community resources. She focuses on helping patients access the appropriate resources and ensuring that they receive comprehensive care tailored to their specific needs.
