Blake Terwilliger, BA, Behavioral Health Case Manager , plays an essential role in supporting incarcerated individuals as they prepare for successful reintegration into the community. Beginning 90 days prior to release, Blake provides case management focused on improving post-release outcomes by addressing key barriers to stability and promoting long-term success.
Blake works closely with individuals during incarceration to develop personalized reintegration plans that include housing assistance, employment support, and connection to critical community resources. He prioritizes eliminating obstacles to accessing medical and mental health care, recognizing that continuity of treatment is vital for a successful transition.
Through proactive planning and coordination with correctional staff, community providers, and support networks, Blake ensures that individuals leave incarceration with the tools and connections they need to rebuild their lives. His work promotes reduced recidivism, enhanced public safety, and increased access to care and opportunity.
