Caroline Hardwick, Targeted Case Manager, is dedicated to helping individuals and families access the resources and support they need to thrive. She works closely with patients, caregivers, healthcare providers, and community partners to coordinate services and ensure each person’s unique needs are met.

Through individualized service planning, advocacy, and resource coordination, Caroline helps connect individuals to medical, educational, social, and community support. Her compassionate, person-centered approach helps patients and families navigate challenges, access needed resources, and work toward their goals with confidence.
