Angela Wooten, BA, a certified Targeted Case Manager (TCM), is here to provide individualized support in both school and clinic settings, working closely with families, educators, healthcare professionals, and community providers to ensure children and adults receive the medical, educational, social, and community services they need to thrive.


Angela focuses on assessing needs, developing individualized service plans, and coordinating access to vital resources. Whether working with students navigating special education and therapy services or adults managing chronic conditions and behavioral health needs, Angela is committed to providing person-centered, goal-directed services that reduce barriers and promote long-term success. Angela is a graduate of Glasgow High School and Western Kentucky University. She has been serving our community for over 20 years.
