Family Medical Centers Honored with Six National Awards for Excellence in Patient Care, Innovation, and Advocacy

Family Medical Centers is proud to announce that we have been recognized with six national awards from the Health Resources and Services Administration (HRSA) for exceptional performance in patient care, clinical outcomes, innovation, and advocacy. These honors are based on the most recent Universal Data System (UDS) results and reflect the hard work and dedication of our entire team across Kentucky’s rural communities.


2025 Awards Received:

  • Health Center Quality Leader – Gold: Recognizing outstanding overall performance in clinical quality measures.
  • National Quality Leader – Behavioral Health: Meeting and exceeding national benchmarks for promoting behavioral health and wellness.
  • National Quality Leader – Heart Health: Excellence in preventing and managing cardiovascular disease.
  • National Quality Leader – Diabetes: High-quality screening, treatment, and support for patients living with diabetes.
  • Advancing Health Information Technology (IT) for Quality: Leveraging EHR systems, telehealth services, and clinical data sharing to improve patient care and engagement.
  • Advocacy Center of Excellence – Gold (designation active for two years): Demonstrating a strong, ongoing commitment to advocating for accessible, equitable healthcare.
These recognitions are more than just awards—they are a testament to our mission. Since our founding in 2007, Family Medical Centers has worked tirelessly to provide quality, affordable healthcare to some of Kentucky’s most rural and underserved areas. Today, we serve patients across 33 counties, offering state-of-the-art medical services, compassionate care, and community-focused programs that go above and beyond traditional healthcare.
