How to Share Your Feedback

We offer several ways for you to share your thoughts and experiences with us:

  • Online Feedback Form: You may submit a specific complaint or compliment to our staff at this link.
  • In-Person: During your visit, you may speak to the clinic manager to provide feedback about your experience.
  • Phone: Call our patient services team at 270-858-6655, Option 6, between 8:00 AM and 4:00 PM CT, Monday through Friday.
  • Email: Send us your feedback at
  • Mail: Address your written feedback to Cumberland Family Medical Center, Inc., PO Box 2399, Russell Springs, KY 42642.
