Family Medical Centers (FMC) is proud of our commitment to exceptional care and service as recognized by the multiple, prestigious Health Center Awards. These accolades reflect our relentless dedication to improving health outcomes, advancing equity, and ensuring every patient receives the quality care they deserve.

HRSA Quality Awards for 2025

Based on the most recent Universal Data System (UDS), health centers that achieve high levels of quality in service and clinical performance based on patient results are recognized with various levels of quality awards by the Health Resource Service Administration. For the last reporting period, Family Medical Centers was recognized in five areas as listed below out of over 1400 community health centers nationwide.

Health Center Quality Leader Gold Badge: best overall CQM (clinical quality measure) performance among health centers. This award was awarded only to 136 health centers in the nation.

National Quality Leader 2025 Behavioral Health: meets or exceeds national benchmarks for one or more clinical quality measures that promote behavioral health.  Only 173 health centers were recognized for this achievement.

National Quality Leader 2025 Heart Health: 222 health centers earned this award for outstanding care in preventing and managing cardiovascular disease.  

National Quality Leader 2025 Diabetes: 187 health centers were recognized for their efforts in excellence in screening, treatment, and support for patients and diabetes.

Advancing Health Information Technology (IT) for Quality: EHR capabilities, telehealth services offered, exchange of clinical information online with key providers’ health care settings, engaged patients through health IT, and collected data on social risk factors.

Grant Funding Awards

Established in 2007, Family Medical Centers (FMC) quickly positioned themselves as the area’s premier location for primary care. Driven by the extreme need for quality, affordable health care in some of Kentucky’s most rural and underserved areas, FMC continues to expand to provide accessible health care for residents across their 33-county federally designated service area.

In order to maximize all revenue streams, FMC aggressively seeks funding from both private and federal agencies through grant applications to further their reach and services. As a result of successful grant applications, keen fiscal management, and diligence in reporting, FMC remains a leader in grant dollars obtained while ensuring that our grant-dollars-per-patient remains low. Good stewardship of our funding streams allows FMC to grow, serve, and expand to provide more care to more people every day. Since our inception in 2007, FMC has successfully applied for, been awarded, and managed approximately 50 grant awards ranging from $4.7 million to $25,000.

Currently, FMC has seven (7) active federal grant awards.

Community Health Center Funding – Family Medical Centers received their first community health center New Access Point designation in 2007 which allowed the establishment of Cumberland Family Medical Center, Inc. At the time, Family Medical Centers (FMC) had two locations and 23 staff members. Since then, FMC has continued to expand their federally designated service area through the process of applying for additional community health center New Access Point opportunities. As a result, FMC has been awarded six (6) community health center awards totaling $6,341,365 to establish health centers in underserved areas.

Expanded Hours Grant – This $1 million award ($500,000 each year for a two-year project period) provides support to extend the hours of operation at our Glasgow Pediatric Healthy Kids Clinic location. As a result, this site is now open an additional 15 hours per week to better serve our pediatric population at this regional health center. From early mornings, to evenings, to weekends, FMC is available to care for our smallest, most vulnerable patients.

Behavioral Health Service Expansion – This award provides FMC with funding to expand our behavioral health services to create increased and easier access to care for this patient population. This $1.1 million dollar award provides $500,000 a year for a two-year project period with an additional $100,000 in YR 1 for minor alterations/renovations. Through this project, FMC has hired additional behavioral health care professionals, expanded behavioral health care options, and added Substance Use Disorder (SUD) services. Alterations are being completed at one of our Promise Community Health Centers to create enhanced access and maximize space to allow for easier patient access to services.

Early Childhood Development – This $400,000 award is for a two-year project allows for the improvement of developmental outcomes among children by increasing access to as well as increasing the number of developmental screenings provided for children ages 0 – five as early childhood development expertise is building into our current care teams.

School Based Health Service Sites - This original $400,000 grant for a two-year project period has since expanded with additional funding for a third year as well as continuation of funding that is annually applied to our community health center award. The original project allowed FMC to create school-based health centers within the schools located in the Frankfort Independent School District in Frankfort, KY.

Advanced Nursing Education – Nurse Practitioner Residency Fellowship Program – This award affords FMC the distinction of being the first and only Nurse Practitioner Residency Program in the state of Kentucky. The initial project is for four years and brings $2.8 million to the program for the creation, implementation, and operation of the NP residency program. This continues to be an extremely successful project and has to date graduated eight residents since its inception allowing them the extra skills, knowledge base, and confidence base to successfully enter the workforce in a very competitive, fast-paced field.


Primary Care HIV Prevention (PCHP) – Our PCHP award allows FMC to support HIV activities throughout our service area with a focus on our targeted patient population consisting of those living at or below 200% of the Federal Poverty Level while residing in geographically isolated rural communities. The project focuses on the direct hire of primary care and/or enabling service providers who support the delivery of integrated primary and HIV care services, linkage to treatment, and care coordination necessary for persons who test positive for HIV, including internal and external referrals for appropriate treatment. This is a $700,000 award that also includes an additional $37,000 one-time funding supplement.

Quality Improvement Fund – Transitions in Care for Justice-Involved Populations – A $1 million award, this two-year project partners FMC with a county detention center and other community partners to provide transitional services to local and state detainees who are within 90 days of release from detention. The mission of this project is to ensure a smooth transition back into the community through direct assistance with Social Determinants of Health as well as health care coverage and services including behavioral health and Substance Use Disorder (SUD) as appropriate.

Quality – UDS+ Award – This supplemental funding is designated for the acquisition of new equipment, meeting interoperability standards, and providing data management training to health center staff. This award totals $103,207 for a one-year project period.

Advocacy in Action

National Association of Community Health Centers (NACHC) Gold Level Advocacy Center of Excellence - Family Medical Centers was one of the first health centers in the nation to be recognized as an Advocacy Center of Excellence (ACE) by NACHC. This designation recognizes health centers that create a culture of advocacy within their organization to ensure policymakers at all levels of government commit to supporting the affordable, equitable, and innovative care that health centers provide. There are three ACE designations—Bronze, Silver, and Gold—that distinguish tiered levels of consistent engagement and demonstrate an ongoing commitment to making advocacy an organizational priority. FMC has been recognized as a Gold Level Advocate since the inception of the recognition program.

Advocates for Community Health (ACH) Membership - Advocates for Community Health (ACH) is a nonpartisan, non-profit advocacy organization dedicated to advancing healthcare access through our nationals largest federally qualified health centers (FQHCs). Focused on the needs unique to the largest FQHCs, ACH prioritizes advocacy on issues such as sustainable funding, the 340B Drug Pricing Program, the protection of Medicaid for CHCs, workforce expansion, value-based care, and healthcare innovation. Family Medical Centers has been an ACH member since 2021.
